The National Philanthropy Day online submission process is hosted by BlackTie South Florida. By using this method, you will be able to complete the sponsorship process online. Once you set up an account with an email address and password, you will be able to save your work and complete the form at your convenience.  Please note that in using this link you will be directed to a multi-purpose registration page.  The Sponsorship options are clearly labeled on the form. 

Download PDF Form and Mail   
By using this method, you will be able to download the Sponsorship Form as a PDF, complete it electronically, print it out from your computer and send it in via US mail along with your payment.
